If you are interested in attending Dr. Kay’s Reiki Level II with Chakra Flow Energy Medicine Training and you have been previously Certified Reiki Level I with another instructor or lineage… we request that you please fill out this information prior to registering for this class and we will follow up with you.

*You do NOT have to fill out this form if you were certified by Dr. Kay in her Reiki Level I class.
